• This is a prescription medication. All new patients must complete a $45 nurse practitioner clearance visit, valid for 12 months.

This visit:

• Ensures treatment safety

• Confirms medical eligibility

• Covers you for a full year

• Ongoing patients may have future annual clearance visits included at the provider’s discretion.
